Steps for completing an application for a Patriot Service Dog

Before you apply understand Patriot Service Dogs, our training, and our requirements.

Patriot Service Dogs is a small organization, dedicated to quality and community. Our highly individualized approach to training and creating veteran/service dog teams does not suit every veteran. Before applying, please get to know PSD.

Steps to Apply:

Step 1: Get to know Patriot Service Dogs

Along with reading through all parts of this website, it is also helpful to subscribe to our digital newsletter, and follow us on social media.

Step 2: Read the Eligibility Requirements

Completing the application is a process, so please don’t go through that work before reviewing our eligibility requirements. We don’t want to waste your time or get your hopes up

Step 3: Download the Application

A PDF copy of the application can be found below. PSD will not mail you a copy. If you need help with downloading and printing, we recommend using the services at your local library.

Step 4: Complete All Parts of the Application

There are three sections of the application—one for the applicant, one for a family member, and one for a medical provider. All must be completed before PSD will review your application. The “family” section can be completed by anyone in your close support system.

Step 5: Mail in the Application

Yes, it needs to be mailed. The application asks for personal information we do not want to store online.

Step 6: Be Patient

PSD reviews applications quarterly. Your application might come in the day before an application review meeting and you may hear from us the next day, or you might miss that meeting by a day. The schedule of these meetings is not public.

Step 7: Understand What Your Application Means

Meeting the eligibility and completing an application does not mean you will receive a service dog from PSD. It is a step in a process that might end in a service dog match.
